This form is to be filled out by prospective leaders who want to bring a mission trip team of at least 5 people to serve with Global Frontier Missions in Oaxaca, Mexico. If you are an individual or a member of a team of 4 or less people, please fill out the Mission Trip Application for Individuals.

We are currently only able to accept applicants from the United States and Canada.
